Indemnification And Hold Harmless Agreement-Signature Line
At the end of the above agreement are five lines. They are as follows:
Name of the Organization spelled out
By:
Name:
Its:
Date:
The First seems self explanatory.
The second is the one that is confusing me. It appears to me to be redundant of the first line. Or is it just that simple?
The third is clear enough and the forth I believe is the organizational position of the person named in line four. The last (5th) is also self explanatory.

You asked about a signature block for an organization.
Organizations (corporations, partnerships, LLCs, etc.) can only act through natural persons. And natural persons do not want to become liable for the acts of an organization for which they are merely a/the tool for transacting. Therefor, the second line is the signature of the person, followed by the typed name of the signator and his/her title or capacity with that organization.
Signing with such a signature block limits a signator from being sued when the organizations defaults.
