We usually use the short form for all the powers of attorney. There’s no reason to be more elaborate. It becomes durable by checking the right box on the form.
While it is fairly simple, it is still a mistake to do this yourself without the assistance of a lawyer. Any number of simple mistakes, such as an X in the wrong box, can completely change the meaning of the document.
Most of the time my fee is $100. That’s not a whole lot considering that not having it right could cost many thousands.
